That’s why we design a tailored approach for every family, guided by an evidence-based, standardized assessment tool that identifies areas of need and highlights existing parental strengths.

🔧 Case Management Model

Our Case Management Plan offers continued support and advocacy to help families raise their children in a safe, nurturing, and abuse-free environment. Each plan is customized to meet the unique needs of the family and includes:
